Pre-launch notice: Unified Healing Co. is not yet an operating medical clinic. The terms below apply to our website and interest-list forms only. These terms will be updated to include clinical practice policies, financial policies, and patient agreements once the clinic is operational and accepting patients.
Agreement to Terms
By accessing or using the Unified Healing Co. website at unifiedhealingco.com (the "Site"), you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, you should not use the Site.
These terms apply to all visitors, users, and individuals who submit information through our interest forms.
Description of Services
Unified Healing Co. is a ketamine and Spravato therapy clinic currently in the pre-launch phase, preparing to open in Yakima County, Washington. The Site currently serves as an informational resource and interest-gathering tool only.
The Site provides general information about ketamine therapy, Spravato treatment, and the conditions we plan to treat. The Site also offers forms through which prospective patients and referring healthcare providers can express interest in our services.
The Site does not provide medical advice. Nothing on this website constitutes medical advice, diagnosis, or treatment. The information provided is for general informational purposes only and should not be relied upon as a substitute for professional medical advice from a qualified healthcare provider. Always seek the advice of your physician or other qualified healthcare provider with any questions you may have regarding a medical condition.
No Doctor-Patient Relationship
Use of this Site, including submission of an interest form, does not establish a doctor-patient relationship between you and Unified Healing Co., its owners, employees, or affiliates. A doctor-patient relationship is established only after a formal intake process, clinical evaluation, and mutual agreement to proceed with treatment, which are not available through this Site at this time.
Submitting an interest form does not guarantee that you will be accepted as a patient, that treatment will be available to you, or that your insurance will be accepted.
Interest Forms and Information Submission
By submitting information through our interest forms, you represent that:

You consent to being contacted by Unified Healing Co. via the email address you provide regarding your inquiry. Telephone calls may be made when appropriate to respond to your inquiry. SMS text messaging requires separate, affirmative consent as described below under "Communications"

You understand that submitting an interest form does not constitute a request for medical treatment and does not create any obligation on the part of Unified Healing Co.
You may request removal from our interest list at any time by using the unsubscribe link provided in our emails or by contacting us directly.
Communications
By submitting your contact information, you agree to receive email communications from Unified Healing Co.. These communications may include confirmation of your inquiry, periodic updates about our clinic's progress toward opening, and notification when scheduling becomes available.
We will not send unsolicited marketing unrelated to Unified Healing Co.'s services, and we will never sell or share your contact information. With your separate, explicit consent given through a dedicated opt-in checkbox, we may also send marketing or promotional communications about our services by email. You may opt out of email communications at any time by using the unsubscribe link provided in the email or by contacting us directly.
Providing a phone number on our forms does not, by itself, enroll you in SMS text messaging. We only send SMS messages to individuals who separately and affirmatively consent by checking a dedicated, unchecked SMS opt-in checkbox on our forms. The checkbox is unchecked by default and is presented separately from the phone number field. SMS consent is optional and is not required to submit an interest form, request information, or receive services from Unified Healing Co. By checking that box, you confirm you are at least 18 years of age or the age of majority in your jurisdiction, and you agree to receive non-marketing SMS messages from Unified Heart Co LLC dba Unified Healing Co related to your inquiry, clinic opening updates, notification when scheduling becomes available, appointment and scheduling communications, intake or administrative reminders, and other service-related communications you request. For referring healthcare providers, messages may also include referral partnership inquiries, referral coordination, and related administrative communications.
We do not use SMS to send marketing or promotional communications. Any marketing or promotional communications are handled separately through email and require separate consent where applicable.
You may cancel SMS messages at any time by replying STOP to any message you receive from us. We will send a one-time confirmation that you have been unsubscribed; after that confirmation, you will no longer receive SMS messages from us unless you affirmatively opt in again. If you experience an issue with our messaging program, reply HELP for assistance or contact us at aaron@unifiedhealingco.com. Carriers are not liable for delayed or undelivered messages. If any provision of these terms is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect.

Unified Healing Co. reserves the right to modify these Terms and Conditions at any time. Changes will be posted on this page with an updated effective date. Your continued use of the Site following any changes constitutes your acceptance of the revised terms.

Once Unified Healing Co. begins clinical operations, these terms will be supplemented with additional policies including:

Financial policies: insurance billing, co-payment requirements, self-pay rates, payment methods, and payment plan options

Clinical practice policies: initial evaluation procedures, treatment protocols, medication policies, referral requirements, confidentiality and HIPAA practices, applicable Washington consumer health data practices, and minor consent requirements

Insurance policies: accepted carriers, pre-authorization requirements, non-covered services, and patient financial responsibility
