Pre-launch notice: Unified Healing Co. is not yet an operating medical clinic and is not yet a HIPAA covered entity. We are currently collecting interest-list information only (name, email, phone, zip code, and general treatment interest), including some health-related information you choose to share, such as conditions or treatments you're interested in. This may qualify as "consumer health data" under the Washington My Health My Data Act; see Washington Consumer Health Data below for details. This privacy policy will be updated prior to the clinic opening to reflect our full HIPAA Notice of Privacy Practices once clinical operations begin.
About this Policy
This Privacy Policy describes how your personal information may be used and disclosed by Unified Healing Co., and how you can access and control your information. Please review it carefully.
Unified Healing Co. is not yet a HIPAA covered entity. Once the clinic is operational and treating patients, this page will be replaced with a full HIPAA Notice of Privacy Practices, as required by the Privacy Regulations promulgated pursuant to the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Information We Currently Collect
During the pre-launch phase, our website collects the following information through our interest forms:

Contact information: first name, last name, email address, phone number

Insurance carrier or payment preference (carrier name only, no policy numbers or member IDs)

For referring providers, we also collect: professional credentials, specialty, practice name, practice location, estimated number of patients who might benefit from treatment, and referral partnership interest.
What we do NOT collect at this time: We do not collect Social Security numbers, insurance policy numbers, member IDs, dates of birth, medical record numbers, diagnostic codes, prescription information, or insurance card images. Some of what we do collect, such as your general treatment interest or antidepressant history, is still health-related information; see Washington Consumer Health Data below for how we handle it. Our interest forms are not a medical consultation, intake process, or request for treatment.
How We Use Your Information
Information collected through our interest forms is used solely for the following purposes:

To confirm receipt of your inquiry and communicate with you about our clinic's opening

To understand geographic demand and treatment interest in the communities we plan to serve

We will never use your information for unsolicited marketing unrelated to Unified Healing Co.'s services. We will never sell, rent, or share your personal information with third parties for their marketing purposes.
How We Store and Protect Your Information
Information Sharing and Disclosure
Your information is stored within our customer relationship management (CRM) platform, which uses industry-standard encryption in transit (TLS/SSL) and at rest. Access to your information is restricted to authorized members of our team only.
While no method of electronic transmission or storage is completely secure, we take reasonable precautions to protect your personal information from unauthorized access, use, or disclosure.
We do not share your personal information with third parties except in the following limited circumstances:

Service providers: We may share information with service providers who assist us in operating our website and managing communications (e.g., email delivery services, CRM platform). These providers are contractually obligated to protect your information and use it only for the services they provide to us.

SMS and mobile opt-in data: No mobile information, including SMS opt-in data and consent, is shared with third parties or affiliates for marketing or promotional purposes. We may share this information with subcontractors who provide support services on our behalf, such as customer service, under the same confidentiality obligations described above. Outside of these limited circumstances, text messaging originator opt-in data and consent will not be shared with any third party.
Your Rights
You have the following rights regarding your personal information:

Withdraw consent: Where we rely on your consent to process your information, you may withdraw it at any time, by replying STOP to opt out of text messages, using the unsubscribe link in marketing emails, or contacting us using the information below.
SMS Text Messaging
Providing your phone number on our forms does not, by itself, sign you up for SMS text messages. We only send SMS text messages to individuals who have separately and affirmatively agreed to receive them by checking a dedicated SMS consent checkbox on our forms. This checkbox is unchecked by default and is presented apart from the phone number field itself, so entering a phone number without also checking that box does not enroll you in text messaging. SMS consent is optional and is not required to submit an interest form, request information, or receive services from Unified Healing Co.
By checking that box, you consent to receive non-marketing SMS text messages from Unified Heart Co LLC dba Unified Healing Co related to your inquiry, clinic opening updates, notification when scheduling becomes available, appointment and scheduling communications, intake or administrative reminders, and other service-related communications you request (or, for referring providers, referral partnership inquiries and coordination). Message frequency may vary. Message and data rates may apply.
We do not use SMS to send marketing or promotional communications. Any marketing or promotional communications are handled separately through email and require separate consent where applicable.
You can opt out of SMS messages at any time by replying STOP to any message you receive from us, or reply HELP for assistance. No mobile information, including SMS opt-in data and consent, will be sold, rented, or shared with third parties or affiliates for their own marketing or promotional purposes. We may disclose this information to service providers who process communications on our behalf, such as our CRM and messaging platform providers, who are contractually limited to using it only to provide those services. Outside of these circumstances, text messaging originator opt-in data and consent will not be shared with any third party.
Cookies and Website Analytics
Our website may use cookies and analytics tools to understand how visitors interact with our pages. These tools collect anonymous, aggregated data such as page views, time on page, and referral sources. We do not use cookies to collect personally identifiable information. You may disable cookies in your browser settings at any time.
Children's Privacy
Our website and interest forms are not intended for individuals under the age of 18. We do not knowingly collect personal information from minors. If you believe a minor has submitted information through our forms, please contact us and we will promptly delete that information.
Washington Consumer Health Data
Some of the information collected through our interest forms, including your conditions of interest, treatment types of interest, antidepressant treatment history, veteran status, and anticipated treatment timeline, may qualify as "consumer health data" under the Washington My Health My Data Act, even though it is not protected health information under HIPAA at this stage.
Categories we collect: general treatment and condition interest, antidepressant treatment history, veteran status, and anticipated treatment timeline, as described above under Information We Currently Collect. Source: directly from you, through our website interest forms. Purpose: to respond to your inquiry, understand demand for specific treatments in the communities we plan to serve, and prepare for your potential care.
We do not sell consumer health data, and we do not share it with third parties for their own marketing or advertising purposes. We may share it with service providers, such as our CRM platform, who are contractually limited to using it only to provide services to us.
If you are a Washington resident, you have the right to confirm whether we are collecting your consumer health data, to access it, to request its deletion (including asking us to notify any service providers who received it to also delete it), and to withdraw any consent you've given for its collection or sharing. You also have the right to know what personal information we collect generally and to opt out of the sale of your personal information; we do not sell personal information. To exercise any of these rights, contact us using the information below.
We do not use geofencing technology to identify, track, collect data from, or send notifications to individuals near any healthcare facility.
Future HIPAA Notice of Privacy Practices
Once Unified Healing Co. begins clinical operations, this page will be updated to include our full HIPAA Notice of Privacy Practices. That notice will describe how your protected health information (PHI) may be used and disclosed for treatment, payment, and healthcare operations, and will detail your rights under HIPAA including:
